PBM’s do a lousy job sourcing best pricing as it’s not in their best financial interests. More plan sponsors are realizing sourcing is the key to lower prescription drug costs and are ditching their PBM.

Finding alternate sources for best prices is not rocket science. Anyone can do that. So we did and here is a hint of what you can do too:

Two retail brick & mortar Texas pharmacies provide low cost generic drugs as much as half the price of PBM procurement. A Canadian pharmacy offers high cost specialty drugs and brand name drugs for as much as 80% less delivered to your doorstep within 3 days. Here’s some examples of savings:

 1.    Sovaldi (sofosbuvir) – Gilead’s hepatitis C treatment was priced at $1,000 per pill in the U.S. (totaling $84,000 for a standard course), while in India, it was available for as little as $4.29 per pill—a 99.5% price difference. 

    2.    Emflaza (deflazacort) – Used for Duchenne muscular dystrophy, this drug was priced at $89,000 per year in the U.S., compared to approximately $1,000 annually in Canada and the UK—a 98.9% difference. 

    3.    Truvada (emtricitabine/tenofovir) – An HIV prevention drug costing up to $2,000 per month in the U.S., while in other countries, it is often priced under $100—a 95%+ disparity. 

    4.    Acthar Gel (corticotropin) – Mallinckrodt’s treatment for infantile spasms and multiple sclerosis was priced at over $34,000 per vial in the U.S., up from $40 in 2001, while alternative treatments abroad cost significantly less. 

    5.    Humira (adalimumab) – An autoimmune disease drug costing over $5,000 per month in the U.S., while in the UK, it is available for approximately $1,000—a 80% difference.

6. Mounjaro‘s list price in the US is $1080.  India pays $50.
