It has long been known that a small portion of the population accounts for a disproportionately large share of health care spending, with 20 percent of the population typically accounting for
80 percent of total health care spending.

80% have claims less than $5,000, 50% have claims less than $1,000. So why not build a plan based on utilitarianism, the moral system founded by English philosopher Jeremy Bentham (1748–1832) and encapsulated in the principle of “the greatest good for the greatest number.” Seems plan sponsors are doing the opposite.
