FORREST GUMP HOSPITAL AGREEMENT
This Agreement is entered into this ______day of______________, 2025 between ABC HOSPITAL and the self-funded benefit plan covering eligible employees and dependents of ABC Fence Company (“AFC”).
ABC HOSPITAL will accept Assignment of Benefits for services provided to employees and dependents of (AFC) covered by the Plan.
The Plan waives all patient financial responsibility for covered services.
This Agreement may be terminated by either party at any time. Any charges incurred prior to the termination date of this Agreement will be paid in accordance with the terms of this Agreement.
