HOW IT WORKS

The employer makes a fixed monthly contribution of $15, $30, or $50 for all eligible employees. Employees pick and choose the plans that fit them best. Employees get guaranteed-issue underwriting for all available products with the majority of industry-normal participation requirements waived for all.

  • Basic Life: $100,000 / $100,000 AD&D
  • Voluntary Term Life: $100,000 / $100,000 AD&D
  • Voluntary Dental (Six plans up to $2,500 maximum benefit)
  • Voluntary Short-term Disability (up to 60%)
  • Voluntary Long-term Disability (60%)
  • Voluntary Vision (Exam / Lenses / Frames / Contacts)
