Two Hospitals Blocks Apart, Same % of Medicare = Very Different Payment

The figures above use Hospital-with-Supplementals, the layer most people mean by “% of Medicare.” Bellevue is a public safety-net hospital. Its DSH and uncompensated care.

Two NYC Hospitals @ 200% Medicare:

  • NYU: $51,562.
  • Bellevue: $107,457.

Same Sepsis DRG. 5 blocks apart. Both are correct. Both are “Medicare.”

NYU Langone (550 1st Ave) and Bellevue Hospital (462 First Ave) sit on the same avenue. Same FY2026 IPPS Final Rule. Same MS-DRG 871 (sepsis w/MCC). Same methodology.

A $55,895 gap on the same admission.

“Medicare” is not one number. It is at least five, depending on the definition:

-National (DRG weight × CMS standardized amount)

  • Market (wage-adjusted to the NYC CBSA)
  • Hospital (adds DSH + IME + Capital)
  • Hospital with supplementals (adds UCP, VBP, HRR, HAC, EHR, bundled)
  • Claim-modeled (adds real outliers on the real claim)
